Completing this form adds you to a list of people who have said “yes” to helping Cityside community members who are in need of some sort of assistance. Usually, this assistance will be in the form of a ride—to service or to a medical or some other type of appointment. Perhaps someone may request help with a small household project that they cannot do on their own. By signing up to be a Community of Care Member you do not commit yourself to any number of hours nor are you required to accept any request for help you may receive. You are simply indicating your willingness to help out if and when you can.

In addition to your contact information, please indicate what area of the city you live in. We will try to send requests from people needing assistance to people who live in the same general area that may be able to help. Thank you for your willingness to serve your Cityside Community in this way.
